Choose the week of camp you want to attend, promote it, and get your teens excited. Use Camp promo DVD. 2. Access camper and counselor forms by clicking the Fun n Son Logo above and print the following: Application Form (Camper and Counselor) Information Packet : (Camper Contract Form pg. 6, Activities Permission Form pg. 7, Medical Release Form pg. 8) 3. Mail appropriate completed forms along with one church check for camper and counselor deposits, made out to Joplin District NYI, to Rick Pearson at the address on the forms by May 1st. 4. Start raising funds as early in the year as possible. 5. Send final payment for campers and counselors attending. Mail one church check made out to Joplin District NYI, to Rick Pearson at the address on the forms by June 1st. If not possible, contact Fred Toomey for payment arrangements. 6.
